Description:

The Aging and Disability Resource Center of Eagle Country - Baraboo Office provides information and assistance, early intervention & prevention services, brief medication management assessments/services and memory screening. The ADRC assists the general public but focuses on adults w/disabilities and the elderly. Call or stop in during office hours.

The ADRC is the intake point for Family Care and the IRIS Waiver program. Will also work with young adults with disabilities and their families to help them understand their options as they transition out of school and into the adult world.

The ADRC also provides access to Benefits Specialists. Our Disability and Elder Benefit Specialists provide information, assistance and representation to Wisconsin residents age 59 and under as well as those 60 and over, with benefit and entitlement programs such as Social Security, Supplemental Security Income, Medicare, Medical Assistance, Food Share, and the Wisconsin Community Options Program (COP).

Transportation

The Sauk County Commission on Aging provides a "volunteer Driver Escort Service" for elderly 60 and older and those with a disability who do not receive support or assistance from family, friends, community agencies, or organizations for their transportation needs. Persons must be able to enter and exit private vehicles with only the assistance of an arm. Wheelchair passengers must be capable of making an independent transfer. Commission on aging does have vehicles that can transport persons who are wheelchair bound. We do not transport within the city limits if the city has a taxi service. A cost may be associated with this.

Fixed Route Bus Service

Bus route connecting Reedsburg, Lake Delton, Baraboo, Merrimac, Bluffview, Prairie du Sac, and Sauk City. Operates 8:00am - 5:00pm Monday-Friday excluding holidays.

Paid Drivers

These are paid drivers for Sauk County. They drive the 10 passenger van, and 5 passenger van. On a fixed route. Both have a wheelchair lift to transport persons in a wheelchair. These drivers also do "Miles of Smiles" trips.

Caregiver Connection

The national caregiver support program is designed to give caregivers a rest. The program provides:

- Information to caregivers about available services
- Assistance to caregivers about available services
- Respite care to enable caregivers to be temporarily relieved
- Supplemental services, on limited basis, to complete care needed
-Services for grandparents or relative caregivers

Living Well with Chronic Conditions

Program consists of workshops which meet once a week for six weeks. Participants in the program discover skills and techniques which may be used in addition to the health care they receive to manage the disease. Through this course you will learn how to manage symptoms, communicate more effectively, lessen frustration, fight fatigue and make daily tasks easier.

News Magazine

A bi-monthly magazine which is sent upon request to Sauk County resident who are over the age of 60, veterans, or adults with disabilities.

Homestead Tax Credit

AARP tax aides are available January-April to assist in completing the WI Homestead Tax Credit forms. Appointments should be scheduled through the ADRC.

Helping Hands at Home

A worker referral list for Sauk County residents and their families who request household help for indoor chores, outdoor chores and respite care. Workers on the list are not employed nor supervised by the ADRC.

The ADRC will help connect you to volunteer opportunities available in the community.

Notary Public

The ADRC has a Notary Public on staff.
