Description:
HealthNet operates a free clinic that provides general, primary Medical Care to residents of Rock County with no health insurance, Medicare, Medicaid or Medical Assistance and low household incomes. Flu shots are also available at the clinic.
In order to receive services (or continue receiving services) from HealthNet, patients are required to go through an enrollment interview to determine eligibility. For enrollment dates and time either call 608-756-4638 or visit the website www.healthnet-rock.org. Appointments are not accepted and individuals are seen on a first come first serve basis.
Remember to bring ALL DOCUMENTS with you!! If you are missing documents, you will NOT be able to re-enroll and will be asked to return another time. For example, if you live with your parents and they provide assistance (let you live rent-free, pay your bills, pay for food, etc.) we need their financial documents (pay stubs, taxes, proof of other income sources, etc.).

2.Income verification
a.Payroll check stubs (two of the most recent for every working member of the household). If self employed, Schedule C
b.Unemployment or Social Security verification documentation (cannot be a bank statement)
c.Current verification of all other sources of income (child support, rental assistance, workers compensation, disability, alimony, pension & annuities, interest & dividends, financial assistance for living expenses)
3.Current tax information
a.1040 EZ (page 1)
b.1040 (pages 1-2)
c.If self-employed, Schedule C (pages 1-2)
4.Other documentation to provide
a.If you have applied for an insurance hardship exemption, please provide proof.
5.Must apply for BadgerCare/Medicaid
a.Visit https://access.wisconsin.gov to complete the enrollment process.
b.Please bring the letter of approval or denial that states your full name.
HealthNet also offers dental care to people who qualify for their assistance. The service is offered at 1344 Creston Park Dr. However, clients must first be seen at the main office to complete the enrollment process and then are referred to the Creston Park dental clinic. You do not need to enroll if you are utilizing the Forward Health card at HealthNet’s dental clinic. Medicaid/Badgercare is now accepted at the Dental Clinic.
