Description:
Palliative care is a specialty that focuses on the relief of suffering and the improvement of the quality of life across the spectrum of an illness. An interdisciplinary team supports the physical, spiritual and emotional needs of the patient and family. - The team includes the patient's physician, nurses, certified nurse's aides, social workers, chaplains, and volunteers. The team provides assistance with pain and symptom management. Palliative services are offered in patient's home, community-based residential facility (CBRF), residential care apartment complex (RCAC) or during hospitalization.
Eligibility:Patients of any age facing a serious illness.
- Patient who may be receiving curative treatment including chemotherapy, radiation or dialysis.
- Patients with a prognosis greater than six months and hospice care is not appropriate.
- Patients needing or requesting support to manage healthcare needs.
