Cedar Crest, Inc - Health Care Center is licensed by the State of Wisconsin as a 24-hour skilled nursing care facility. It is Medicare and Medicaid certified. Residents within our Cedar Crest family have priority access to all other centers of our retirement living. Residents receive services 24 hours per day. Visiting hours are 8:00am - 4:30pm. Eligibility: Over age 55 and must meet Nursing Home level of care. Accepts MA, Medicare, self pay and some insurance. Appointment preferred for tour of facility.
Cedar Crest, Inc - Health Care Center provides 24-hour skilled nursing care in a state-of-the-art environment. You'll find lovely decorated corridors coming off a beautifully indoor park and extending into intimate café areas. Immediately outside rooms, living and dining areas provide added visiting space for residents and their families. All rooms are private with bath. Residents are encouraged to decorate with personal treasured home accessories and furniture. Cedar Crest, Inc operates on the person first philosophy.
Cedar Crest, Inc - Assisted Living provides residents with the supportive services they need in the comfort and privacy of their own, personal living space. Private studios are furnished with each resident’s personal belongings and are located adjacent to special family and dining areas within a setting designed to enhance lifestyle.
Cedar Crest, Inc - Memory Care Program offers residents carefully planned daily programs and activities to provide socialization and structure, while helping residents feel a sense of independence and value. Residents are encouraged to participate in individual and group programs designed to assist them to reach their full potential.

Cedar Crest also has Townhomes and apartments available. A variety of payment schedules are available and vary by the square footage and location of the home. Residents of apartments have access to assistance with ADL's including medication management. Apartment home residents are members of the Cedar Crest community and enjoy full access to all facilities, activities, and services, including priority access to assisted living, memory support, and the health center, should the need arise.
Pet Policy: Dog or Cat under 40#s, additional pet deposit of $500.00. Pets are only allowed to residents in the Apartments and Town Homes.
Cedar Crest is a 24/7 facility with business office hours of Monday - Friday 8:30am - 4:30pm.
