Description:
Hospice is a service provided to terminally ill patients and their families. They must be under the care of a physician and have elected palliative care vs. active treatment for their disease.
Home Care is a service provided in the home under orders of a physician.
Service Areas: Illinois and Wisconsin.
Home Health Care:
Monroe Clinic Home Care is a service provided in the home under the orders of a physician. Call for appointment and information about services. Services include, skilled nursing visits, PT, Speech therapy, OT, social workers and home health aides. Call for an appointment and information on services. Each Service has a different fee. Interpreters available.
Eligibility: Must be home-bound and in need of intermittent health care. Skilled services require a physicians order.
