Description:

Basic medical care for individuals without insurance or are under-insured. Individuals need to be at least 185% of federal poverty level to receive treatment. Individuals will be further referred to other programs/services that they may be eligible for.

TO BE ELIGIBLE YOU MUST:
* Be a resident of Marinette or Menominee (MI) Counties
* Be a US citizen
* Be between the ages of 19 to 64 years old
* Meet low-income financial guidelines
* Not have any insurance, including Medicare/Medicaid, County Social Services, * VA Benefits
* Provide proof that you cannot afford employer-based and/or Marketplace- based insurance plans.

Basic health care. Limited pharmaceuticals. Referrals for additional medical and community-based services.



The Clinic may pay up to 250.00 per patient per year for medications or as deemed necessary.



If people need dental care they will give them a voucher for a local dentist.



Intake: May call Director before going to be sure they qualify and reserve their spot to be seen.



Hours: 2 Tuesday's of the month (weeks vary).
Registration @ 5:30 p.m. Appointments Required. (No incoming calls during clinic hours) - Located on the 2nd floor of the Aurora Medical Office Building, 4061 Old Peshtigo Rd., adjacent to Urgent Care Dept.



Proof of residency and income are required.
