Care management services consist of professionals (Social Worker or Registered Nurse) who specialize in assisting older adults and their families with long-term care services. Care management integrates health, home and community services assisting older adults and families with long-term care needs. They possess knowledge about costs, quality, availability of services and can assist or arrange in obtaining services a person is assessed for or as needed.
Referrals for My Choice Family Care Case Management for individuals 60 years or older must come through the Aging Resource Center of Milwaukee County by calling 414-289-6874 or for disabled individuals 18 - 59 years old through the Disability Resource Center of Milwaukee County at 414-289-6660.
