Description:
Center for Veterans Issues (CVI) - VETS Place Central (VPC) offers transitional housing to 72 veterans, single-room rental units to 16 age-eligible or disabled veterans and day services to veterans who meet eligibility requirements.
Eligibility requirements:
Veteran must be homeless or at risk of becoming homeless at time of initial contact
Must be motivated to work and/or enroll in education training programs
Medical and mental health condition must be stable
Must comply with program policies and procedures
Must be willing to work with case manager and establish individual plan
Must commit to alcohol and drug-free living
Services include:
Addiction therapy for individual or group
Adult nurse practitioner services including a history and physical
Health prevention and wellness education
Management of acute and chronic illness
Case management
Anger management and assertiveness training
Job service training
Life skills training in communication and coping skills of daily living
Psychiatric therapy and treatment planning
Diagnostic assessment
Medication management
Veterans benefits and entitlement counseling.
